SICK CALL TREATMENT LOGA log referred to as the Sick Call Treatment Log ismaintained for each ship or activity. The log containseach patient’s reporting date and time, name, rate,social security number, command, division,complaint, diagnosis, treatment, disposition, anddeparture time from sick call. When full, the log isretired in accordance with SECNAVINST 5212.5.BINNACLE LISTThe Binnacle List, NAVMED 6320/18, is used toexcuse an individual from duty for a period of 24 hoursor less. This report is prepared by the senior medicaldepartment representative on board and should besubmitted to the commanding officer no later than0930 each day. This form contains a list of individualsrecommended to be excused from duty because ofillness. The list is approved by the commandingofficer, and no names may be added without the CO’spermission.MORNING REPORT OF THE SICKThe Morning Report of the Sick, NAVMED6320/19, is used to excuse an individual from duty for aperiod of more than 24 hours. This report contains alist of the sick and injured, including names,diagnoses, and conditions. It is prepared by the seniormedical department representative on board and issubmitted to the commanding officer by 1000 daily.When it is necessary to excuse someone from dutyafter the Morning Report of the Sick is submitted, addthe patient’s name to the Binnacle List, and submit theappropriate report to the commanding officer. If apatient is still unfit for duty when the next MorningReport of the Sick is submitted, add his name to theNAVMED 6320/19 as of the date on which his namewas first entered on the Binnacle List. If a satisfactorydiagnosis cannot be established, simply note“Diagnosis undetermined” and indicate the chiefcomplaint. Report suspected cases of malingering tothe commanding officer.TRAINING LOGAll lectures and training periods that are part of themedical training program should be recorded in theTraining Log and a notation made in the MedicalDepartment Journal. The entries should include thedate, location, type of training (GMT, etc.) or subjectmatter, and what department personnel received thetraining (Engineering, Deck, etc.).IMMUNIZATION LOGTo aid you in annotating health records and fillingout monthly medical reports, develop and maintain animmunizations log. As the minimum, the informationshould include the date; name; rank; social securitynumber; immunization type; duty station; and, forpersonnel receiving PPDs, a contact phone number.There should also be space for adverse reactions.WATER TEST LOGThe purpose of the water test log is to record thereadings of daily residual chlorine or bromine levelsand the weekly bacteriological examinations requiredon potable water aboard ship and in the field.APPOINTMENT LOGThe purpose of the appointment log is to trackmedical consultations and clinical appointments thatare scheduled by the Medical Department. When apatient is unable to keep an appointment, a notationindicating both the cancellation and rescheduling ofthe appointment should be made in the log. Multipleappointment cancellations by the same member shouldbe brought to the attention of the member’s chain ofcommand.DIRECTIVES ISSUANCE SYSTEMLEARNING OBJECTIVE: Recall thepolicies and procedures for maintainingdirectives, drafting correspondence, andfiling.As a Hospital Corpsman in an administrativebillet, you may be responsible for maintaining yourcommand’s files and the CD-ROM library of Navydirectives.
